Carers’ experiences of hospital discharge - Summer 2021

Our report explores the impact of hospital admission and discharge on the wellbeing and outcomes of patients and their carers between Summer 2020 and late Spring 2021

This joing project with Action for Carers Surrey was undertaken to investigate the experience of Discharge to Assess from the perspective of the carers of those admitted to hospital.

Key findings

  • Poor communication between ward and carers
  • Carers unable to communicate a patient’s baseline to those providing treatment or arranging discharge
  • Contradictory, confusing information
  • Poor levels of inclusion in post-discharge planning with carers not prepared for discharge
  • Poor post-discharge signposting and support.
