Your experience of eye (sight) tests

  • Do you have regular sight (eye) tests?
  • Does anything stop you having these tests?

Our team are particularly keen to hear feedback from members of black and Asian minoritised communities.

As well as going out to speak to people, we are also running a survey so that we can hear the experiences of as many people as possible.

This survey is running online (link below) and there are also paper copies available. If you need a paper copy or would prefer to share your experience in your own words please get in touch with our Helpdesk.

5 people who complete the survey before the closing date of 28 November will be selected at random to receive a £10 'One 4 All' gift card.

NHS Health Checks

As part of our engagements for this quarter, we are out in the community asking people aged 40 - 74 whether they have been invited to an NHS Health Check and what their thoughts are about them.

If you are aged 40 - 74, whether you have had a NHS Health Check or not, we would like to hear from you.

To try and hear from as many people as possible, we are also running a survey. This can be completed online (link below) or you can get in touch with our Helpdesk if you need a paper copy or would prefer to share your experience in your own words.

5 people who complete the survey before the closing date of 31 December will be selected at random to receive a £10 Love 2 Shop voucher.

GP practices - changes to meet your needs

  • Do you have a disability or impairment?
  • Do you need a language translator?

As part of our Primary Care priority this quarter, we are out in the community talking to people who need reasonable adjustments or language translators at their GP practice (Reasonable adjustments are a change that a service provider must make to remove or reduce barriers faced by people with a disability or impairment).

We would like to hear if you have asked for changes to be made to help you use services at your GP practice and how well your GP practice is meeting your needs, or whether you need these changes but have never asked for them.

You can also share your views via our survey. 5 people who complete the survey before the closing date of 31 December will be selected at random to receive a £10 "One 4 All" gift card. The survey can be completed online (link below) and paper copies are also available. If you need a paper copy or would prefer to share your experience in your own words, please get in touch with our Helpdesk.

NHS App and patient feedback

Do you use the NHS App?
If yes, what do you like about it (and what could be better)
If not, why not?
Either way, we’d love to hear from you.

As part of our Involvement in People priority, we are seeking people's thoughts about the NHS App - whether people use it, what they use it for, or why they don't use it.

If you live in Surrey, please complete this survey (closes 31 December 2025). This can be done online (link below) or please get in touch with our Helpdesk if you need a paper copy, or would prefer to share your experience in your own words.
