Healthwatch Surrey wants to hear from people who need reasonable adjustments or language translators at their GP practice.

Reasonable adjustments are a change that a service provider must make to remove or reduce barriers faced by a disabled person. 

We would like to hear your experience - whether you have asked for changes to be made to help you use services at your GP practice and how well your GP practice is meeting your needs. Or whether you need these changes but have never asked for them.

5 people who complete the survey before the closing date of 31 December will be selected at random to receive a £10 voucher.
