Surrey Immunisation Survey for Parents

Surrey County Council (SCC) Public Health Team in collaboration with Surrey Heartlands Integrated Care System (ICS) would like to invite you to take part in a short survey to explore parental attitudes to immunisation.

In this survey, they hope to better understand your views on vaccinating your children and explore why some parents/guardian may or may not choose to get their children immunised against infectious diseases. The results of the survey will feed into local work aiming to improve uptake of childhood immunisation.

Some of the questions in the survey cover sensitive topics, and Surrey County Council would like to reassure you that all answers will be confidential and they will not be able to trace your answers back to you or your child/children.

The survey should take less than 15 minutes to complete and contains 22 questions. Please spend a few minutes to complete the survey and contribute to this important research.
