Barriers to access and experience of sight (eye) tests for school-aged children – survey closes 27 June
We would love to hear your feedback about the barriers to access and experience of sight (eye) tests for your school age child/ren (aged 4 to 16 years).
Healthwatch Surrey champions the voice of local people to shape, improve and get the best from NHS, health and social care services. We are independent and have statutory powers to make sure decision makers listen to the experiences of local people.
Healthwatch Surrey is seeking to understand the barriers that may prevent you from accessing sight tests for your child and, if you have taken your child for a sight test, we would like to hear about the experience.
Responses to this survey are anonymous. The closing date for the survey is 27 June.
5 people who complete the survey before 27 June will be selected at random to receive a £10 ‘One 4 All’ gift card. Those selected will be notified within 1 month of the close of the survey.
